At CAAC, we know every situation is different, so we have two options for completing paperwork.
To speed up the check-in process and ensure that you are highly satisfied with your experience in our office, we kindly request that you complete the following online patient paperwork prior to your first visit. If you complete the online forms, once you click SUBMIT, your forms will be delivered to the clinic you select.
If you are filling out paperwork online, please use either a computer or tablet and NOT a cell phone. Also, we recommend using the latest version of Google Chrome or Mozilla Firefox browsers.
If you’d rather, you can complete the fillable PDF forms. Please print them out and bring them with you to your visit. Plan on arriving 15 minutes early.
Please consult our medication alert as you may be asked to hold certain medications for 7 days prior to your appointment.
