What to Expect at Your First Appointment

CHECK IN / PAPERWORK

Please fill out new patient forms prior to your first appointment. You will also need to bring a current photo ID and a current insurance card.

You have several options when filling out your new patient forms:

1.) You can fill out the form electronically within your web browser, just make sure to complete on a desktop or laptop computer and press submit at the end. It will be delivered directly to your preferred location.
2.) You can download the blank form to your computer to fill out -either electronically or by hand- and print at a later time. (DO NOT fill out the form electronically and then try to download it. It will only download a copy of the blank form.)

Please do not print forms two-sided. We are unable to separate the pages we need from the documents that go in the front of your file if you do so.

APPOINTMENT

New Patient appointments generally include a thorough medical history, examination, and testing. The time and cost of this visit will vary based on your symptoms and the testing that your allergist recommends. Generally the evaluation takes between 2-3 hours. Because of this, it is vital to avoid taking antihistamines for 7 days prior to your visit. We will be unable to perform testing if you have taken an antihistamine within 7 days of your appointment.

Based on the evaluation, the doctor will formulate a diagnosis and recommend treatment options. Education regarding your diagnosis and treatment plan is an important part of your initial visit. We will work with you to design a treatment program which results in the most effective therapy for your medical problems. Our goal is to arrive at a treatment plan which allows you to take control of your asthma or allergy issues.

At any time during your visit, please feel free to ask questions if there is something that you do not understand.

PAYMENT

Please note that payment is expected at the time of service. For information about insurance processing and payments, please visit our insurance page.

Appointment Cancellations

Please call the office as soon as possible if you are unable to keep your scheduled appointment. Twenty four hour notice is needed for cancellations. There may be a cancellation fee of $75 for appointments that are missed or cancelled with less than 24 hours notice.
