When you click the link below, the forms will open in a new tab. You can print and/or download using the icons in the upper right hand corner of the screen. You have several options when filling out these forms:
1.) You can fill out the form electronically within your web browser and then print it.
2.) You can download the blank form to your computer to fill out -either electronically or by hand- and print at a later time. (DO NOT fill out the form electronically and then try to download it. It will only download a copy of the blank form.)
3.) If you have a browser or viewer that doesn’t support electronic forms, you can print the forms from within the tab and fill them out by hand

Please call the office as soon as possible if you are unable to keep your scheduled appointment. Twenty four hour notice is needed for cancellations. There may be a cancellation fee of $50 for appointments that are missed or cancelled with less than 24 hours notice.

What to Expect at Your First Appointment
New Patient appointments generally include a thorough medical history, examination, and testing. The time and cost of this visit will vary based on your symptoms and the testing that your allergist recommends. Generally the evaluation takes between 2-3 hours.
Based on the evaluation, the doctor will formulate a diagnosis and recommend treatment options. Education regarding your diagnosis and treatment plan is an important part of your initial visit. We will work with you to design a treatment program which results in the most effective therapy for your medical problems. Our goal is to arrive at a treatment plan which allows you to take control of your asthma or allergy issues.
At any time during your visit, please feel free to ask questions if there is something that you do not understand.
