Appointment Instructions


Office Visit Details for Established Patients:

  • Please arrive 15 minutes early to your appointment
  • Bring your current insurance card and photo ID
  • Your copay or deductible will be collected
  • If your insurance requires a referral to see a specialist, please bring it to your appointment
  • If you have referral questions, please contact your primary care physician or insurance company
  • A $75 cancellation fee may be charged if your appointment is not canceled with in 24 hours
  • If you have not seen us within the last year, please fill out these forms and disclosures.

Additional Instructions for New Patients:

  • Please have your new patient paperwork completed 24 hours in advance of your visit
  • Avoid taking your antihistimines for seven days prior to your appointment
  • Please plan on the appointment being approximately 2 hours long

The following instructions only apply to patients who have made an appointment for these specific tests.

Additional Instructions for Pulmonary Function / Full Lung Test:

  • Refer to your handout instructions for specific medications to be taken or to be held
  • Test will take approximately 2 hours

Additional Instructions Exercise Challenge:

  • Wear comfortable clothing and appropriate shoes
